The Maui Police Department shared the following instructions for providing DNA samples from close link relatives of persons missing in Maui.
1. Collect a BUCCAL swab:
- Collect three (3) separate swabs.
- Six (6) up-down cycles inside each side cheek pushing hard enough to the outside that the cheek stretches out a bit. Mouth should be clear of any food, smoking materials, etc. for about 10 min prior to collection.
2. Complete the “Family Reference Sample Collection Form:” <Download form through this link>
3. Ship the DNA sample and completed form to:

4. Email the following information for the person missing in Maui to This email address is being protected from spambots. You need JavaScript enabled to view it.:
- Name
- Date of Birth
- Address
- Phone Number
- Height & Weight
- Last known whereabouts
- Tattoos and any other unique identifiers
- Also include the DNA sample shipment tracking number
Those based in Maui may provide their DNA sample through the Family Assistance Center located at the Hyatt Regency’s Monarchy Ballroom.
